Once you have scheduled an appointment, submit the New Client Information forms. You can save time if you fill them out in advance of your first appointment. If you are unable to fill out the forms in advance, we suggest you arrive at least 10 – 15 minutes prior to your appointment so you can complete them before your appointment.
